If you have your grandmother's birth name, and she was born before June 1880 (and was still living in the orphanage at that time), you should be able to find her in the census. Then, if the institution's name isn't written on the census page, you can look up the address in a directory.
I found an ancestor who was born at the Home for Friendless Children on Webster Street via the (at that time) laborious task of going through the 1880 census page by microfilmed page over a period of time. I had her birth date and the approximate location but not the birth name (imagine the frustration!). Once I found the home, there were only two babies born in the right month there, so I then followed their lives as best as I could through records, finding that one of them later married (right name and age, anyway). I was able to discount her and settle with about 90% confidence on the other's being the right gal. It can be done! (Usually ...)
